Shawn Casey doesn't work for me. I get a "500 Internal Server Error" page.

On another message board it was noted that that page has an offer for CVS on there that you have to say yes to and put in a user name and password for in order to get to the next page.

Thanks! I tried this and it worked. BTW...how many times can survey say "Last Page"? I have counted at least 10 so far on this one!
