Entries must be received by 06/23/08 and winners will be drawn on or around 07/11/08.

Sweepstakes is open to residents of the 50 United States and the District of Columbia, and you must be 18 years of age or older. Only one entry per person allowed.

There is a spot at the top of the Entry Form to input a UPC code, but this is not required, nor is the "tip" box. Just proceed as normal and your entry will go through (I read this in the rules).
