I noticed there are two Symbicort offers, I completed and received credit on one, but not the other. I wanted to know if they are the same offer or different, which then I can do the other one.

The web page takes me to a pregnancy site but there is Symbicort ad on the top right.

Is that what we are suppose to fill out?

They look to be two different offers but go to the same page in the end. So I would say not to do it if you have already requested information on Symbicort before just to be safe.
