I was a member of QPB club about 5 years ago. Would that affect me getting credit if I sign up again through SR?

The possibility exists. If it is something you want to do again I would sign up through SR and just know that the possibility exists that you won't get credit. If the club doesn't really matter then I would switch and try Book of the month club or Zooba or one of the other book clubs just to ensure you will get credit and most of them you can get the same books although probably not in paperback.
