Click on the blue Members box and then access the select offer under the easy offer links. They are in alpha order and it's listed under National Smoker's Survey.

The Clubs of America was a quick $6. Its a shopping offer that you can do multiple times but it is similar to a trial offer in my opinion as its a flat $ amount back for a product. Same as MyWinesDirect.com.
