Problems with GF Quick Surveys

Twice tonight in FF and IE I have started a "Quick Survey" through DS + SM. Enter my age, next screen asks for my zip code which I enter. Get an error "please enter a valid street address" which I do and get the same error. No way past it. Tried both browsers. One survey was on cosmetics, the second one on Consumer topics.

Anyone else encounter the same issue?
