I just finished the DVD survey, no problems for me. In fact, it was one of those easy, fun ones! End page is white with the following:
"Congrats!
You've successfully completed the survey!
Felicitations! Vous avez repondu au sondage.
Congratulazioni! Hai completato con successo il sondaggio."
Hope it credits!

I just tried this and got about 13% in when it would not let me go any further. I kept getting an error message at the bottom of the page when I hit next. I tried in IE and FF. Oh well. I guess this was not meant to be!
