I went back to check where on the page I clicked, but for some reason the web page pulling up says "Thank you for visiting this offer. At the moment, you are not eligible for this offer, or the advertiser may not service your area. To visit other offers you may be qualified for, please click HERE." I think it was one of the advertisements on the page, maybe on top or off on the right hand side. Sorry I couldn't be of more help.

I had to scroll all the way to the bottom of the page, there was a long horizontal banner there, about the width of the page content. If it is not there anymore maybe the offer has already been pulled?

Yeah, I'm glad you found it.

I figured Flomax out too. It was just my laptop being weird (which it has a tendency to do):computer:. I switched to my desktop, and there was the form to fill out! Now I'm keeping my fingers crossed that it credits.
