Inappropriate Samplicous Survey

I was trying a survey and it asked if I had a child under 18 avalable to take the survey, I answered yes. It asked the age and gender and I answered 13 male, then it said the following questions are for your 13 year old. It asked a couple of routine questions, then it got into health questions, salary questions, how many people work at your firm (with answers from 1 to I don't know) THEN it kept asking about medical questions. and then it went into have you ever had erectile dysfunction? trouble getting an erection...etc
These questions DO NOT belong in a survey for children. I do not have the survey invite, but I did make a copy of the URL for these questions:

I am not looking for credit or anything else. They just need to REMOVE this survey as one directed at children. If I could have found a link to report it to them directly I would have. I am hoping you can pass this information along.
