Exact same thing happened to me about a week ago.

think it is time for bed took SM and finished for it to freeze up and put me through another one after I already finished

Completed a SM about on line recruitment companies and at the end it went to one of those companies website and not back to SR;I know that won't credit.

Completed a DS about a large electronics retailers rewards program and ended up with:

I just got an SSI which was about ereaders and tablets. At the end it was really to sign-up for an online community and went to their website and did not return to SR. It did not credit.

DH just finished the same survey. Did not route back to the SSI completion page and did not receive credit.
