I just tried for the OTX, and this is the message i got after i entered my info and clicked "start survey":

Possible cause: Server was unable to process request. --> Object reference not set to an instance of an object.
Please click the back button and try again.

i've clicked the back button as it says (twice) but i still get the same page...
